Root Cause Analysis Program

The ultimate goal of Root Cause Analysis (RCA) is to eliminate problems by removing the actual cause that would create the problem in the first place. Safety issues are evaluated to determine what root cause created the situation. Once the root cause is isolated, analysis can suggest a corrective action or a series of actions to be implemented to eliminate the root cause thus eliminating that safety issue from occuring again.
